Terance "Terry" Jerome Kozminski of Lynchburg, OH. Passed away on November 4, 2024, at the age of 66. He was born in Grand Rapids, MI in 1958 to Delores and Jerome Kozminski.

Terry is survived by his beloved wife of 44 years, Susan D. (nee Neuman) Kozminski; his daughter Kate M. (Nicholas) Strilka and daughter Cari A. (Robert) Elam; four loving grandchildren, Connor Elam, Everett Elam, Eleanor Elam and Nicholas "Max" Strilka; one sister, Mary Ann (Wally) Melching; also survived by numerous nieces, nephews, in-laws, relatives and many dear friends. Terry is preceded in death by his parents, Jerome and Delores (nee Mroczkowski) Kozminski; one sister, Stella Saunders.

Terry proudly served as a Corpsman in the United States Navy attached to the First Marine Division. In addition to his career in ophthalmology, he was a musician and avid gardener. He loved traveling, dining and being with his grandkids.

A Catholic mass will be held at 10:30 AM on Friday, November 15 at St. Patrick Church, 130 Stone Alley, Fayetteville, OH 45118. Celebration of Life immediately following mass in the church reception hall. In lieu of flowers, donations may be made in memory of Terry Kozminski to Leukemia & Lymphoma Society (lls.org) or Patient Advocate Foundation (copays.org/donors).
