Falk Sr. Loving mother of Marie (Fred) Hargis, Frank Jr. (Lise) Falk and Susan (Ron) Anderson. Caring grandmother of Douglas Horton, Alissa Horton, Nicholas Horton, Renee Anderson and Michelle Nachazel. Proud great-grandmother of 13 and great-great grandmother of 3. Dear sister of 6 late sisters and 4 late brothers. Cherished daughter of the late William J. and Pearl Mae (Pursinger) Fairbanks. Also survived by numerous nieces, nephews and many dear friends. Services will be held on Wednesday, June 11, 11 AM, at St. Thomas Episcopal Church, 100 Miami Ave., Terrace Park, OH. 45174, where friends will be received from 10 AM until the time of service. Interment, Goshen Cemetery. Darren Elin of St. Thomas Episcopal Church will be the minister for Juanita's services.
