Jackie "Jack" Ray Brizendine, 63, of Clarksville, Ohio passed away on March 29, 2016. He was the beloved husband of 24 years to Helen Brizendine; loving father of Jack Ray Brizendine, Jr., Amy (Jeremi) Brizendine-Parker, Chloe Brizendine, Joyce Morgan, and Sandy Webb; proud grandfather of Corissa Morgan, CJ (Chris) Morgan, Gregory Webb, Garrett Webb and Jordan Parker and great-grandfather of Jaydelyn Webb; dear brother of Anna Charles; also leaving behind many friends and family. Jack was preceded in death by his parents, Jack and Allie Brizendine and seven siblings. A funeral service will be held on Friday, April 1, 2016 at 7pm at Tufts Schildmeyer Family Funeral Home, 120 W.
