Passed away April 16, 2016 at the age of 86. Hazel was an active member of Maineville United Methodist Church and a Retired LPN from Bethesda North Hospital. A Celebration of Life service will be held on Saturday, April 23 at 11 AM at Maineville United Methodist Church, 60 E. Foster-Maineville Rd, Maineville.
